    Scheduling and Intake Paperwork:

    My office staff will call you to schedule an appointment for your first consultation and give you instructions on how to complete my health history and assessment. They will, also, give you instructions of how to send any recent (in the last 6 months) laboratory tests and other pertinent records to our office before your first consultation. (We must receive the intake paperwork, labs, and reports before the first appointment. If we do not have the necessary paperwork 24 hours before your scheduled appointment, we will cancel your appointment.)

    First Consultation:

    At your first consultation appointment, which will be a 60 minute appointment, we will discuss your intake paperwork and go over your health history, diet and lifestyle. Necessary blood work and other Functional Medicine testing will be discussed, as well as, a plan for diet and lifestyle changes. Those recommendations will need to implemented by you immediately, as well as, getting the recommended labs completed as soon as possible. Any questions you have will be answered at this appointment.

    After this appointment and each subsequent appointment, you will receive a written summary of what was discussed in the call. The summary will include a plan of action: testing that is recommended, supplements, diet and lifestyle recommendations.

    Report of Findings:

    The report of findings will be a 60 minute appointment. We will review the initial lab testing, any supplements recommended and additional diet and lifestyle modification. If necessary, additional lab testing will be recommended. All of you questions will be answered.

    Ongoing Treatment

    After the above steps are completed, a follow-up appointment will be scheduled approximately six weeks from the report of findings appointment. We will review progress, any follow-up test results, and make recommendations to the treatment plan. If any additional retesting of labs are necessary, they will be recommended.
